8.3.16  ACFI 12 - COMPLEX HEALTH CARE

 

Data from the following Assessment populates directly into the Complex Health Care Checklist within a resident’s ACFI Claim Submission:

 

 

The answers selected in the linked questions auto-populate the Complex Health Care Checklist.

Details from other relevant Assessments also populate into the Evidence section of ACFI 12 Complex Health Care, displaying the assessment from which the data is linked and the relevant data, as recorded in the assessment.  This ensures that users can view any inconsistencies that may exist between details in other sections of the program.  The following assessments contain questions which are linked to the Evidence section of ACFI 12 Complex Health Care:

 

·         Admission: Health Management / Physical Exam

·         Health Management

·         Skin Assessment

·         Functional Assessment

·         Admission: Dietary / Nutrition

·         Continence Assessment – post 3-4 Day Assessment

·         Continence Urinary / Bowel – pre-3-4 Day Assessment

Directives may be included in an assessment provided the assessment has been signed by the mandated professional e.g. Registered Nurse for pressure area care. Assessments can then be printed for inclusion in the ACFI pack. Alternatively, organisations may wish to use the Acute Care Plan to record directives.
